Download the Potential Relying Site Study Team Survey. After completing, contact Pedi-PROOVIT to initiate your SMART IRB approval.

Participating InstitutionsYour IRB will receive a notification from SMART IRB regarding your request to rely on UTHealth Houston IRB. Typically, you will need to submit the necessary paperwork, including the protocol and Case Report Form (CRF), to your local IRB to inform them about the study. This step is crucial before they can approve the request in SMART IRB. If you haven't used SMART IRB before, it's advisable to check with your IRB office to understand their specific requirements for approving a SMART IRB request.

Complete the Executed Data Use Agreement (DUA)
After we receive the approval of your SMART IRB, please reach back out to us via email. We will send the template DUA after completing step 2.
Setting up a UTHealth Houston Guest Account
After completing step 3, please contact us to gain access to our UTHealth Houston REDCap. At least 1 person at your institution will need to sign up for a guest account at UTHealth Houston to use REDCap for data entry.
Start Submitting Data via RedCAP
You are now ready to submit data! Please take a look at the following section to help you center start submitting patient data.
RedCAPAll data for Pedi PROOVIT will be stored on REDCap. To input information into REDCap, each person at our institution needs to obtain a UTHealth Houston guest account.
1. Provide Information: Send the names and email addresses of all individuals who will be entering data into REDCap to pediPROOVIT@uth.tmc.edu.
2. Follow Email Instructions: Each person will receive an email with instructions for filling out an online form. Be mindful of the expiration date on the email. If anyone needs the email to be resent, they should contact pediPROOVIT@uth.tmc.edu.
3. Receive Approval: Once approved for a guest account, you will receive an email with your username.
4. Set Up Account: Use the provided link, https://redcap.uth.tmc.edu, to log in and set up your account.
5. Add Study: After setting up your REDCap account, the study will be added to your account. If anyone has questions about the REDCap process, they should contact pediPROOVIT@uth.tmc.edu.
